Big Papa's Okie Toke — Budtender Employment Application

Big Papa's LLC · 4100 W 32nd St S, Muskogee, OK 74401 · Form revised Nov 6, 2026

Print clearly in black ink. Answer every question — write "N/A" if it does not apply. Big Papa's LLC is an equal opportunity employer. A criminal record does not automatically disqualify you; each record is reviewed for what it was, how long ago it was, and how it relates to this job, except where state or federal law bars employment (see Section 7).

Are you at least 18 years old?
Are you legally eligible to work in the United States?

The credential application requires a name-based OSBI background check dated within 30 days of submission, a National Background Check attestation, a photo no older than 6 months, and a government photo ID.

7. Criminal History Disclosure — DEA Rules

Big Papa's handles controlled substances, so DEA's employee-screening rules apply: 21 CFR § 1301.90 (screening questions) and 21 CFR § 1301.76(a) (a registrant may not employ, in a job with access to controlled substances, anyone convicted of a felony relating to controlled substances, or anyone who has had a DEA registration application denied, a DEA registration revoked, or surrendered a DEA registration for cause).

What to list: every criminal charge ever filed against you, in any city, county, state, federal, tribal or military court — whether it was dismissed, deferred, acquitted, convicted, or is still pending.

Do NOT list: (1) any record a court has expunged/sealed — under Oklahoma law (22 O.S. § 19) you may answer as if it never happened; (2) traffic tickets that were not arrests (DUI/DWI must be listed); (3) juvenile offenses, unless you were charged as an adult; (4) military offenses, except general court-martial.

DEA Question 1 (21 CFR § 1301.90): Within the past five years, have you been convicted of a felony, or within the past two years of any misdemeanor, or are you presently formally charged with committing a criminal offense?
DEA Question 2 (21 CFR § 1301.90): In the past three years, have you knowingly used any narcotics, amphetamines or barbiturates other than those prescribed to you by a physician?

Marijuana used under an OMMA patient license is not part of this question (63 O.S. § 425).

DEA Question 3 (21 CFR § 1301.76(a)): Have you EVER been convicted of a felony relating to controlled substances?
DEA Question 4 (21 CFR § 1301.76(a)): Have you EVER had an application for DEA registration denied, a DEA registration revoked, or surrendered a DEA registration for cause?
Question 5: Have you EVER been charged with any criminal offense (including charges that were dismissed, deferred or acquitted)?

If you answered Yes to Question 1, 3 or 5, list every charge below (use the back or an extra page if needed). DEA requires details of the offense, location, date and sentence.

Anything you want us to know about the charges listed (rehabilitation, time passed, circumstances):

I understand that leaving out any charge that is not expunged/sealed, or giving false information, is grounds to withdraw a job offer or end employment, even if discovered later.

8. Authorization & Release — OSBI Background Check

(This page stands on its own. Sign it separately from the application.)

I authorize Big Papa's LLC (Big Papa's Okie Toke), 4100 W 32nd St S, Muskogee, OK 74401, and its owners, to request and receive my criminal history record from the Oklahoma State Bureau of Investigation (OSBI), and to search Oklahoma and other state and federal court records (including OSCN and ODCR), the Oklahoma Medical Marijuana Authority (OMMA), and the Drug Enforcement Administration (DEA), for the purpose of deciding on my employment and keeping it. I authorize Big Papa's LLC to use these results for my OMMA employee credential application and the National Background Check attestation, and to repeat these checks during my employment, including at each yearly OMMA credential renewal.

A copy, scan or photo of this authorization is as valid as the original.

Information needed for the OSBI name-based search (used for the background check only, not for any hiring decision):

9. Applicant Certification

I certify that everything on this application is true and complete. I understand that false statements or omissions are grounds for not hiring me or for ending my employment at any time. I understand that employment with Big Papa's LLC is at-will, and that any job offer depends on my obtaining and keeping an active OMMA employee credential and on the results of the OSBI background check and the DEA screening above, reviewed as stated at the top of this application.
